All applications must be made in person at the Consular Section of the Embassy where the Fingerprinting Card Forms are available.

For first-time applicant (living/working abroad)

1. Secure a Fingerprint Card form from the Philippine Embassy and completely fill-up the personal data (Civilian Form for Filipinos and Alien Form for foreigners)

2. In case you are a married woman, the order of writing your name is as follows;

• Maiden surname (father’s surname)

• Husband’s surname

• First name or given name

• Maternal surname (family name of your mother)

3. For foreigners, attach work permit or Bureau of Immigration ID. Foreign students may attach school records or diplomas. Provide the complete address and reference in the Philippines.

4. For Filipinos, attach valid passport, work permit or valid IDs.

5. Attach a 2×2 colored photo taken within the last six (6) months, with white background

6. Proceed to the police station/agency in your area to have your fingerprints impressed (rolled impression) on the appropriate spaces. The officer/person who assisted you in the fingerprinting should sign his name and indicate his official designation on the space provided for the purpose on the card.

7. Have the Fingerprint Card form authenticated by the Philippine Embassy so that the document can be considered legal for use in the Philippines.

8. The authenticated accomplished form must be sent to the National Bureau of Investigation (NBI).

9. You may submit the form through a representative in the Philippines. Send it along with a signed letter authorizing him/her to do the submission on your behalf. Your representative may visit the office of the Electronic Data Processing Division (EDPD) in NBI Taft Avenue, Manila for further assistance.

10. Upon receipt of your clearance certificate, please check if an NBI DRY SEAL is embossed on its lower left portion. The clearance is invalid without it.
